The rules define practices that are universally recognized and accepted for assuring that organizations consistently understand and meet the needs of their customers.&lt;br /&gt;ISO 9000 is also highly generic. Its principles can be applied to any organization providing any product or service anywhere in the world.&lt;br /&gt;Since meeting customer needs is one of the (many) definitions of quality, ISO 9001 is often called a quality system or a quality management system. But the rules, referred to as requirements, go beyond quality matters as they are traditionally understood. The requirements fall roughly into the following types:&lt;br /&gt;&lt;br /&gt;a. Requirements that help assure that the organizations output (whether product, service, or both) meets customer specifications. (Making, and keeping, them happy.)&lt;br /&gt;&lt;br /&gt;b. Requirements that assure that the quality system is consistently implemented and verifiable. (We must actually do what we say we are supposed to do. This must be verifiable via independent, objective&lt;br /&gt;audit.)&lt;br /&gt;&lt;br /&gt;c. Requirements for practices that measure the effectiveness of various aspects of the system. (In God we trust; all others bring data.)&lt;br /&gt;&lt;br /&gt;d. Requirements that support continuous improvement of the companys&lt;br /&gt;ability to meet customer needs. (We cannot sit still. We must strive to get better all the time, because customers change, and competitors gain strength.)&lt;br /&gt;&lt;br /&gt;Nothing in ISO 9000 is new. The first edition, published by ISO in 1987, was drawn almost word for word from a British quality system standard. It in turn evolved from a long succession of written quality system specifications that had their ultimate origin in the defense and arms industries. Most of the practices required by ISO 9000 have been in use in industries of various kinds for decades. One intent of ISO 9001 is to simplify things for organizations. ISO 9001 strives to harmonize the sometimes conflicting, sometimes redundant quality programs that have traditionally been imposed by major corporations on their suppliers. (Note, however, that ISO 9000 is not meant to supersede customer, legal, or regulatory requirements.)&lt;br /&gt;&lt;br /&gt;Very often, major customers require or strongly suggest that their suppliers implement ISO 9001 systems. Equally often, such customers require independent verification that suppliers are meeting the equirements.&lt;br /&gt;&lt;br /&gt;So third-party registration bodies audit suppliers, confirm compliance to the ISO 9001 standards, and register the suppliers. It does not stop there. To stay registered, suppliers must undergo periodic (often semi-annual) surveillance audits, also carried out by their registration body.&lt;br /&gt;&lt;br /&gt;Implementing an ISO 9001 quality system is neither cheap, nor easy. How costly and difficult it can be depends on:&lt;br /&gt;&lt;br /&gt;a. The level of commitment of senior management. (The single most important factor.)&lt;br /&gt;&lt;br /&gt;b. Where you are when you start. If you have already implemented a disciplined, documented quality system, you will have a less difficult time migrating to ISO 9001. (But that does not mean you will waltz to registration, either.)&lt;br /&gt;&lt;br /&gt;c. Whether your company (or any part of it) is design responsible or not.&lt;br /&gt;&lt;br /&gt;d. How much time you have. If you are under the customers gun and have merely months to get the job done, the process will be highly stressful.&lt;br /&gt;&lt;br /&gt;e. The physical size and configuration of your company.&lt;br /&gt;&lt;br /&gt;The bottom line is this. ISO 9001 is a comprehensive set of rules—a business system, really—that can cause the way your organization runs to profoundly change, almost always for the better. Yet, because it is often customer-mandated, many suppliers regard ISO 9001 as just another hoop to jump through to keep our customers happy.&lt;br /&gt;&lt;br /&gt;&lt;/span&gt;&lt;span class="Apple-style-span" style="font-family: arial, sans-serif; font-size: 13px; line-height: 18px; "&gt;They see their choice as swallow hard, pony up, and jump through the hoops; or walk away from the customer. What many do not fully appreciate is that implementing ISO 9001—expensive, exhausting, and annoying as it can be—can also have the salutary effect of improving the performance of your organization. The awareness training will help reduce fear or resistance to the change that sometimes comes with a large company wide project. Once everyone understands the goals of the ISO project and is ready to be audited, start the gap analysis in sections. Covering sections 4,5,6,7 and 8 all in one audit is a mind melting experience. Start with section 4 to see how fast you can properly document the a section. Then schedule the remaining section based on your experience.&lt;br /&gt;As you go through each section, you will either find that a system is already in place that meets the requirements of the ISO9001 standard or you will write a finding for that section. If you find that the system is in place, simply log the document numbers on the gap analysis sheet and move on. If you find that a large portions of the quality systems are missing, then you can write the equivalent of a major finding with a larger scope and not waste too much time listing every detail of what is missing.&lt;br /&gt;How to use the result of the gap analysis&lt;br /&gt;Once you have completed the gap analysis, you will have a list of missing or under-developed documents, records and systems. If you use the checklist from this web site, the result of the gap analysis will be a list of individual item that must be corrected. If the check sheet said “what document is used to the describe record retention?” and you found no document, then the document should be created. Once you feel that everything is in place, then you will want to repeat the audit to confirm that you can answer every question on the internal audit checklist with a positive response.&lt;br /&gt;If the gap analysis show that your systems are in relatively good shape with some area for improvement, then I would make the gap analysis the first record in your internal audit notebook. This will help build a history of audits. If the gap analysis shows major flaws, you may want to fix them and then perform an internal audit as the first record to meet your internal auditing requirements. An overall link between the systems is often missing which makes the monitoring and the assessment of effectiveness difficult. Double handling of information, contradicting instructions, high maintenance costs, administrative excess and lack of overall transparency are common results.&lt;br /&gt;ISO 9000 Standard Series for Quality (of) Management Systems provide generic guidance for the development of an overall Management System, ISO 14000 provides guidance for Environmental Management, etc. Transparency and monitoring of all business activities can be achieved by integrating all systems into one.&lt;br /&gt;Complaints that ISO 9000 is paralysing operations and, that it does not reflect reality are usually a result of not clearly understanding how the standard can be properly structured to address the needs of a company. ISO 9000 can be structured by focusing on “best practice” process rather than the standard, by fitting the standard to the process and not the process to the standard. Having recognised this, ISO has been working on a new structure for ISO 9000, called “Vision 2000?, taking a process orientated approach to ensure that “best practice” as well as several standards can be addressed within one system. Focusing on process allows the development of a practical “working document”, providing an effective management tool. It is therefore not intended that the &lt;a title="Quality Manual" href="http://www.iso9000-software.com/" rel="nofollow"&gt;quality manual&lt;/a&gt; be a response to the requirements of ISO 9001. As the top-level document describing the management system it is a system description describing how the organization is managed.&lt;br /&gt;Countless &lt;a title="Quality Manuals" href="http://www.quality-template.com/" rel="nofollow"&gt;quality manuals&lt;/a&gt; produced to satisfy ISO 9001:2008, were no more than 20 sections that paraphrased the requirements of the standard. Such documentation adds no value. They are of no use to managers, staff or auditors. Often thought to be useful to customers, organizations would gain no more confidence from customers than would be obtained from their registration certificate.&lt;br /&gt;A description of the management system is necessary as a means of showing how all the processes are interconnected and how they collectively deliver the business outputs. The standard addresses activities that may not be relevant or applicable to an organization. The permissible exclusions are explained in section 1.2 of ISO 9001. Here it states that the organization may only exclude requirements that neither affect the organization’s ability, nor its responsibility to provide product that meets customer and applicable regulatory requirements. The requirements for which exclusion is permitted are limited to those in section 7 of the standard.&lt;br /&gt;Under ISO 9001:2008, it was possible for organizations to exclude functions and processes of their organization that may have been difficult to control or were not part of the order fulfilment cycle. Organizations that designed their own products but not for specific customers could escape bringing these operations into the management system. Marketing was omitted because it operated before placement of order. Accounting, Administration, Maintenance, Publicity, Public Relations and After Sales Support functions were often omitted because there were no requirements in the standard that specifically dealt with such activities. As there is no function in an organization that does not directly or indirectly serve the satisfaction of interested parties, it is unlikely that any function or process will now be excluded from the &lt;a title="Quality Management System" href="http://www.iso14000store.com/" rel="nofollow"&gt;quality management system&lt;/a&gt;.&lt;br /&gt;It is sensible to describe the scope of the &lt;a title="Quality Management System" href="http://www.iso-consults.com/" rel="nofollow"&gt;quality management system&lt;/a&gt; so as to ensure effective communication. The scope of the &lt;a title="Quality Management System" href="http://www.quality-template.com/" rel="nofollow"&gt;quality management system&lt;/a&gt; is one area that generates a lot of misunderstanding particularly when dealing with auditors, consultants and customers. When you claim you have a management system that meets ISO 9001 it could imply that you design, develop, install and service the products you supply, when in fact you may only be a distributor. Why you need to justify specific exclusions is uncertain because it is more practical to justify inclusions.&lt;br /&gt;The scope of the &lt;a title="Quality Management System" href="http://www.iso9001-standard.us/"&gt;quality management system&lt;/a&gt; is the scope of the organization. There is no longer any reason to exclude locations, activities, functions or processes for which there is no requirement in the standard. The reason is because the ISO 9000 family now serves customer satisfaction and is not limited to quality assurance as were the 1994 versions of ISO 9001, ISO 9002 and ISO 9003.&lt;br /&gt;It is not appropriate to address exclusions by inserting pages in the manual corresponding to the sections of the standard and adding justification if not within the scope of the management system – such as ‘We don’t do this!’.&lt;br /&gt;It is much more appropriate to use an appendix as indicated previously in the manual contents list. By describing the nature of the business, you are establishing boundary conditions. In ensuring that the &lt;a title="Quality Policy" href="http://www.iso-consults.com/" rel="nofollow"&gt;quality policy&lt;/a&gt; is appropriate to the purpose of the organization, it must be appropriate to the customers the organization desires to create. It is therefore necessary to establish who the customers are, where the customers are, what they buy or wish to receive and what these customers regard as value. As stated above, the &lt;a title="Quality Policy" href="http://www.iso9000-software.com/" rel="nofollow"&gt;quality policy&lt;/a&gt; is the corporate policy and such policies exist to channel actions and decisions along a path that will fulfil the organization’s purpose and mission. A goal of the organization may be the attainment of ISO 9001 certification and thus a quality policy of meeting the requirements of ISO 9001 would be consistent with such a goal, but goals are not the same as purpose as indicated in the box to the right. Clearly no organization would have ISO 9001 certification as its purpose because certification is not a reason for existence – an objective maybe but not a purpose.&lt;br /&gt;Policies expressed as short catchy phrases such as “to be the best” really do not channel actions and decisions. They become the focus of ridicule when the organization’s fortunes change. There has to be a clear link from mission to &lt;a title="Quality Policy" href="http://www.quality-template.com/" rel="nofollow"&gt;quality policy&lt;/a&gt;.&lt;br /&gt;Policies are not expressed as vague statements or emphatic statements using the words may, should or shall, but clear intentions by use of the words ‘we will’&lt;br /&gt;– thus expressing a commitment or by the words ‘we are, we do, we don’t, we have’ expressing shared beliefs. Very short statements tend to become slogans which people chant but rarely understand the impact on what they do. Their virtue is that they rarely become outdated. Long statements confuse people because they contain too much for them to remember. Their virtue is that they not only define what the company stands for but how it will keep its promises.&lt;br /&gt;In the ISO 9001 definition of &lt;a title="Quality Policy" href="http://www.iso9001-standard.us/"&gt;quality policy&lt;/a&gt; it is suggested that the eight quality management principles be used as a basis for establishing the &lt;a title="Quality Policy" href="http://www.iso9001store.com/" rel="nofollow"&gt;quality policy&lt;/a&gt;.&lt;br /&gt;One of these principles is the Customer Focus principle. By including in the quality policy the intention to identify and satisfy the needs and expectations of customers and other interested parties and the associated strategy by which this will be achieved, this requirement would be fulfilled. The inclusion of the strategy is important because the policy should guide action and decision. Omitting the strategy may not ensure uniformity of approach and direction.&lt;br /&gt;The standard requires that the &lt;a title="Quality Policy" href="http://www.quality-template.com/" rel="nofollow"&gt;quality policy&lt;/a&gt; include a commitment to comply with requirements and continually improve the effectiveness of the quality management system.&lt;br /&gt;A commitment to comply with requirements means that the organization should undertake to meet the requirements of all interested parties. This means meeting the requirements of customer, suppliers, employees, investors, owners and society. Customer requirements are those either specified or implied by customers or determined by the organization and these are dealt with in more detail under clauses 5.2 and 7.2.1. The requirements of employees are those covered by legislation such as access, space, environmental conditions, equal opportunities and maternity leave but also the legislation appropriate to minority groups such as the disabled and any agreements made with unions or other representative bodies. Investors have rights also and these will be addressed in the investment agreements. The requirements of society are those obligations resulting from laws, statutes, regulations etc.&lt;br /&gt;An organization accepts such obligations when it is incorporated as a legal entity, when it accepts orders from customers, when it recruits employees, when it chooses to trade in regulated markets and when it chooses to use or process materials that impact the environment.&lt;br /&gt;The effectiveness of the management system is judged by the extent to which it fulfils its purpose. Therefore improving effectiveness means improving the capability of the management system. Changes to the management system that improve its capability i.e its ability to deliver outputs that satisfy all the interested parties, are a certain types of change and not all management system changes will accomplish this. For this reason, a critical element of the EMS involves identifying and prioritizing the environmental aspects and impacts associated with your facility. An environmental aspect is an element of an organizations activities, products, or services that can interact with the environment. For example, chrome plating is a metal finishing activity and an associated aspect is chrome air emissions.This aspect may have an impact on the environment in several ways, for example, ambient air quality degradation.The &lt;a href="http://www.e-wia.com/" rel="nofollow"&gt;environmental aspects&lt;/a&gt; and impacts of your metal finishing operations can be identified, prioritized, and documented in several ways. Although the written requirements contained in ISO 14001 are straightforward and brief, the level of effort required to conform with the requirements should not be underestimated.&lt;br /&gt;If an environmental review of the facility‘s operations has not been conducted, it is important to retain a qualified consultant to conduct the review. which should focus on the requirements of &lt;a title="ISO 14000" href="http://www.e-wia.com/" rel="nofollow"&gt;ISO 14000 &lt;/a&gt;versus programs currently in use. This activity is known as a Gap Analysis.?E After the Gap Analysis is completed, the environmental status of the operation should be known and the level of effort necessary to develop the &lt;a title="ISO 14000" href="http://www.iso-consults.com/" rel="nofollow"&gt;ISO 14000&lt;/a&gt; program understood.&lt;br /&gt;It can easily take six months to a year to develop a program that meets EMS requirements. Staff should develop the EMS program as they will ultimately be required to manage it. If staff is inadequate to develop the program, a consultant can be retained to provide guidance to staff as the program is being developed.&lt;br /&gt;But, retaining a consultant will not resolve the problem of long term program maintenance. The ultimate goal of developing the EMS is to obtain certification/registration. Aside from the internal benefits offered by the &lt;a title="ISO 14000" href="http://www.iso9001-standard.us/"&gt;ISO 14000 &lt;/a&gt;series of standards, external benefits may be derived through the certification/registration process. The certification/registration process only applies to ISO 14001, and this is the only standard to which the audit process applies. For the purposes of certification/registration, all other &lt;a title="ISO 14000" href="http://www.iso14000store.com/" rel="nofollow"&gt;ISO 14000 standards&lt;/a&gt; are considered guidance. Certification can either be by a self declaration?Eor by an independent registrar.&lt;br /&gt;Obviously the use of an independent registrar would give more credibility to those looking at an organization from outside.&lt;br /&gt;Registrars have individual preferences about how the requirements should be administered. A registrar should be selected early in the process to help ensure that the program being developed is consistent with the registrars preferences. When the program is fully prepared and implemented, the registrar will be notified and a formal program audit undertaken. Unfortunately, it is often the case that ISO 9001 is taken as a means to an end, where the implementation of a QMS is not the primary objective, rather certification is. As a result, SMEs may end up with stacks of documentation waiting to be processed that adds no value, but cost.&lt;br /&gt;According to the requirements of ISO 9001, an organization must develop only six documented procedures: (1) control of documents, (2) control of quality records, (3) internal audits, (4) control of non-conformities, (5) corrective action, and (6) preventative action. A quality manual and several records are also required. The development of other procedures, work instructions, and&lt;br /&gt;other documents is largely at the discretion of the organization. From the very beginning of the process, it is therefore essential that SMEs establish a balanced view between a short-term focus (marketing/sales) and a long-term focus (achieving company-wide quality awareness through TQM). ISO documentation should be considered as an enabler along that way and SMEs must guard against the creation of unnecessary documentation.&lt;br /&gt;However, even when such a view is adopted, many SMEs struggle to move from their initial state to a fully functional &lt;a title="ISO 9001 QMS" href="http://www.e-wia.com/" rel="nofollow"&gt;ISO 9001 QMS&lt;/a&gt;. Over the last several years, we have been involved in ISO 9001 implementation projects in seven different SMEs. The SMEs have ranged in size from approximately 20 employees to 500 employees. The SMEs have been drawn from a variety of sectors in Virginia, including manufacturing, distribution, and services. Based on our experience, we developed a schematic of initial states of an organization in terms of the existence and functionality of the &lt;a title="ISO 9001 QMS" href="http://www.iso14000store.com/" rel="nofollow"&gt;ISO 9001 QMS &lt;/a&gt;. Throughout this paper, existence is equated with the documentation required by the standard while functionality is equated with an effectively operated QMS that leads to increased customer satisfaction and continuous improvement of business results.&lt;br /&gt;A successful QMS must be fully functional and appropriately documented. With that in mind, there are four main states in which SMEs can be located in the beginning of the implementation process:&lt;br /&gt;1. Complete Death: No documentation, no functioning.&lt;br /&gt;This is the state in which there is no indication of the existence and functionality of the QMS. No documentation exists and no processes are in place to help ensure the quality of the product.&lt;br /&gt;Relatively few companies will find themselves in this situation.&lt;br /&gt;2. Informally Alive: No documentation, some level of functioning.&lt;br /&gt;Many SMEs exhibit an organic structure characterized by an absence of standardization and the prevalence of loose and informal working relationships. SMEs operating in this state are more likely to rely on people rather than a system. In such situations, key personnel may resist documentation for two key reasons “(1) documentation is considered a waste of time and (2) documentation of processes and procedures makes the individual less dependable” [2]. SMEs in this state perform some or all of the processes required by ISO 9001 and the QMS may function fairly well. However, they are not willing and ready to document those processes unless there is a cultural change lead by top management.&lt;br /&gt;3. Formally Death: Some level of documentation, no functioning.&lt;br /&gt;SMEs categorized in this state have documented processes and procedures at some degree, however, the documents are generally not followed and do not necessarily reflect the actual manner in which the organization undertakes its operations and management. This situation highlights the fact that the mere existence of documentation does not necessarily lead to a functional QMS. Moreover, such a situation may help perpetuate the view that ISO 9001 is a way for SMEs to market their products and services but that implementation of the standard requires stacks of documents that offer no value.&lt;br /&gt;4. Formally Alive: Some level of documentation, some level of functioning.&lt;br /&gt;Each SME considered in this state, achieves a unique combination of the existence and functionality of processes and procedures that may or may not be required by ISO 9001. Clause 3.8.1 of ISO 9000:2005 defines “objective&lt;br /&gt;evidence” as “data supporting the existence or variety of something” and notes that “objective evidence may be obtained through observation, measurement, test, or other means.”&lt;br /&gt;Objective evidence does not necessarily depend on the existence of documented procedures, records or other documents, except where specifically mentioned in &lt;a title="ISO 9001:2008" href="http://www.iso9000-software.com/" rel="nofollow"&gt;ISO 9001:2008&lt;/a&gt;. The documented quality policy has to be controlled according to the requirements of clause 4.2.3.&lt;br /&gt;Note: Organizations that are revising their quality policy for the first time, or in order to meet the amended requirements in &lt;a title="ISO 9001:2008" href="http://www.iso-consults.com/" rel="nofollow"&gt;ISO 9001:2008&lt;/a&gt;, should pay particular attention to clause 4.2.3 (c), (d) and (g).&lt;br /&gt;Requirements for quality objectives are defined in clause 5.4.1 of &lt;a title="ISO 9001:2008" href="http://www.quality-template.com/" rel="nofollow"&gt;ISO 9001:2008&lt;/a&gt;. These documented quality objectives are also subject to the document control requirements of clause 4.2.3.&lt;br /&gt;b) Quality Manual:&lt;br /&gt;Clause 4.2.2 of &lt;a title="ISO 9001:2008" href="http://www.iso14000store.com/blog" rel="nofollow"&gt;ISO 9001:2008 &lt;/a&gt;specifies the minimum content for a quality manual. The format and structure of the manual is a decision for each organization, and will depend on the organization’s size, culture and complexity. Some organizations may choose to use the quality manual for other purposes besides that of simply documenting the QMS&lt;br /&gt;A small organization may find it appropriate to include the description of its entire QMS within a single manual, including all the documented procedures required by the standard.&lt;br /&gt;Large, multi-national organizations may need several manuals at the global, national or regional level, and a more complex hierarchy of documentation.&lt;br /&gt;The quality manual is a document that has to be controlled in accordance with the requirements of clause 4.2.3.&lt;br /&gt;c) Documented procedures:&lt;br /&gt;&lt;a title="ISO 9001:2008" href="http://www.iso9001-standard.us/"&gt;ISO 9001:2008 &lt;/a&gt;specifically requires the organization to have “documented procedures” for the following six activities:&lt;br /&gt;4.2.3 Control of documents&lt;br /&gt;4.2.4 Control of records&lt;br /&gt;8.2.2 Internal audit&lt;br /&gt;8.3 Control of nonconforming product&lt;br /&gt;8.5.2 Corrective action&lt;br /&gt;8.5.3 Preventive action&lt;br /&gt;These documented procedures have to be controlled in accordance with the requirements of clause 4.2.3 Some organizations may find it convenient to combine the procedure for several activities into a single documented procedure (for example, corrective action and preventive action). Others may choose to document a given activity by using more than one documented procedure (for example, internal audits). Both are acceptable.&lt;br /&gt;Some organizations (particularly larger organizations, or those with more complex processes) may require additional documented procedures (particularly those relating to product realization processes) to implement an effective QMS.&lt;br /&gt;Other organizations may require additional procedures, but the size and/or culture of the organization could enable these to be effectively implemented without necessarily being documented. However, in order to demonstrate compliance with ISO 9001:2008, the organization has to be able to provide objective evidence (not necessarily documented) that its QMS has been effectively implemented.&lt;br /&gt;d) Documents needed by the organization to ensure the effective planning, operation and control of its processes:&lt;br /&gt;In order for an organization to demonstrate the effective implementation of its QMS, it may be necessary to develop documents other than documented procedures. However, the only documents specifically mentioned in ISO 9001:2008 are:&lt;br /&gt;- Quality policy (clause 4.2.1.a)&lt;br /&gt;- Quality objectives (clause 4.2.1.a)&lt;br /&gt;- Quality manual (clause 4.2.1.b)&lt;br /&gt;There are several requirements of ISO 9001:2008 where an organization could add value to its QMS and demonstrate conformity by the preparation of other documents, even though the standard does not specifically require them. Some clauses have not been modified for content but have been rewritten to align &lt;a title="ISO 14001:2004" href="http://www.iso-consults.com/" rel="nofollow"&gt;ISO 14001:2004&lt;/a&gt; with the format, wording, and layout of ISO 9001:2000 and to enhance the compatibility between the two standards.&lt;br /&gt;References in Annex A of the standard are aligned with the numbering in the standard for ease of use. Annex B of the standard identifies similarities and associations between ISO 9001:2000 and &lt;a title="ISO 14001:2004" href="http://www.e-wia.com/" rel="nofollow"&gt;ISO 14001:2004&lt;/a&gt;.&lt;br /&gt;An important change in wording throughout the revised standard appears in requirements that previously stated that an organisation shall “establish and maintain”; these have now been changed to “establish, implement and maintain”.&lt;br /&gt;Throughout the standard the word “personnel” in the original standard is replaced with “persons working for or on behalf of the organisation” in the revised standard. This is included to ensure that external contractors and applicable suppliers are included under the requirements of certain clauses.&lt;br /&gt;In developing, implementing and maintaining the organisation’s EMS, significant environmental aspects, applicable legal requirements and other requirements to which the organisation subscribes must be considered, and management must ensure the availability of resources.&lt;br /&gt;There are additional paragraphs in the introduction, which generally cover:&lt;br /&gt;• the aim of the &lt;a title="ISO 14001:2004" href="http://www.iso9001-standard.us/"&gt;ISO 14001:2004 standard&lt;/a&gt; is to enhance compatibility with ISO 9001:2000;&lt;br /&gt;• alignment is improved between clause references and supporting Annexes. This requirement has been retained in Clause 4.5.2.1 of the revised standard. For example, all organizations implementing &lt;a title="ISO 9001" href="http://www.e-wia.com/" rel="nofollow"&gt;ISO 9001&lt;/a&gt; will need to consider the unique culture within the organization, its size, and the resources available. Beyond those widely discussed points, three issues that merit particular attention are (1) consideration of the QMS as a parallel function, (2) training, and (3) auditing. Key points associated with these issues are discussed below.&lt;br /&gt;Consideration of QMS as Parallel Function&lt;br /&gt;In the case of all of the transitions depicted, real benefits from the QMS are more likely to be experienced if the QMS is implemented directly into the core structure of the organization. SMEs must be cautious against establishing a QMS that is run separately in parallel to its other systems. In SMEs, the parallel subsystem most commonly exhibits itself as a separate Quality Assurance, or in some cases, &lt;a title="ISO 9001" href="http://www.iso14000store.com/" rel="nofollow"&gt;ISO 9001&lt;/a&gt; department. Possible reasons for this may include the existence of rigid departmental boundaries in some SMEs or overemphasis on core activities. As Yauch and Steudel [10] note, SMEs tend to focus their attention on “…necessary routine activities (such as sales, production, shipping, etc.) rather than activities aimed at improving processes or systems.” If a SME insists on establishing a separate quality department, its level of effectiveness can be increased by embedding the QMS in widely-used organizational systems where practical. The integration is largely a function of how well the QMS manages to share information with other subsystems and its ability to align with the policies, norms, goals, and values in place throughout the organization.&lt;br /&gt;Training&lt;br /&gt;In SMEs, training and staff development is more likely to be ad hoc and small scale because of modest human and financial resources and the absence of a specific training budget. To prevent the problems arising from lack of education and training, two things must be done:&lt;br /&gt;1. Education of Top Management: The centralization of decision-making processes within many SMEs means that the management can either be the main stumbling block to change or the main catalyst for change. Therefore, any approach to &lt;a title="ISO 9001" href="http://www.iso-consults.com/" rel="nofollow"&gt;ISO 9001&lt;/a&gt; implementation must involve considerable education for the top management of the organization to create awareness and understanding of the implementation process as a change initiative. Implementing a fully functional and documented QMS requires motivation by top management to appreciate, achieve, and implement the necessary measures to meet the standards’ criteria.&lt;br /&gt;2. Education and Training of Employees: SMEs are often under pressure to quickly gain &lt;a title="ISO 9001" href="http://www.iso9001-standard.us/"&gt;ISO 9001&lt;/a&gt; registration. Meeting the requirements of the standard in a short period of time can prove a formidable obstacle for a small company. Since most SMEs do not possess the needed expertise internally, they may be inclined to hire external experts to provide the necessary technical expertise and manpower. However, having a functioning and documented QMS requires more than that. It requires ensuring that all employees in the organization clearly know what is expected of them and how they can contribute to the attainment of their organizations’ goals. This will likely require the preparation and implementation of a training plan tailored specifically to the unique characteristics and maturity level of the SME.&lt;br /&gt;Auditing&lt;br /&gt;As emphasized throughout the paper, a QMS is not going to produce the expected results unless it is fully functional. While auditing must therefore verify the existence of the necessary documentation, it must also focus on the functionality of the QMS. The measurement of the functionality and the qualitative and financial impacts of a QMS have been the subject of several studies, including Kaynak [11]. Among the categories used to measure functionality and performance improvement, two are particularly noteworthy for our purposes: management commitment and employee involvement. A QMS cannot be functional in the absence of those two characteristics. is not an “upgrade”, and organizations that are certified to ISO 9001:2000 should be afforded the same status as those who have already received a new certificate to &lt;a title="ISO 9001:2008" href="http://www.quality-template.com/" rel="nofollow"&gt;ISO 9001:2008 &lt;/a&gt;No new requirements were introduced in this edition but, in order to benefit from the clarifications of &lt;a title="ISO 9001:2008" href="http://www.iso14000store.com/blog" rel="nofollow"&gt;ISO 9001:2008&lt;/a&gt;, users of the former version will need to take into consideration whether the clarifications introduced have an impact on their current interpretation of ISO 9001:2000, as changes may be necessary to their QMS In order to assist organizations to have a full understanding of the new ISO 9001:2008, it may be useful to have an insight on the revision process, how this revision reflects the inputs received from users of the standard, and the consideration given to benefits and impacts during its development.&lt;br /&gt;Prior to the commencement of a revision (or amendment) to a management system standard, ISO/Guide 72:2001 Guidelines for the justification and development of management system standards recommends that a “Justification Study” is prepared to present a case for the proposed project and that it outlines details of the data and inputs used to support its arguments. In relation to the development of&lt;a title="ISO 9001:2008" href="http://www.e-wia.com/" rel="nofollow"&gt; ISO 9001:2008 &lt;/a&gt;user needs were identified from the following:&lt;br /&gt;- the results of a formal “Systematic Review” on ISO 9001:2000 that was performed by the members of ISO/TC 176/SC2 during 2003-2004&lt;br /&gt;- feedback from the ISO/TC 176/Working Group on “Interpretations”&lt;br /&gt;- the results of an extensive worldwide “User Feedback Survey on ISO 9001 and ISO 9004″ by ISO/TC 176/SC 2/WG 18 and similar national surveys.&lt;br /&gt;The Justification Study identified the need for an amendment, provided that the impact on users would be limited and that changes would only be introduced when there were clear benefits to users.&lt;br /&gt;The key focuses of the ISO 9001:2008 amendment were to enhance the clarity of ISO 9001:2000 and to enhance its compatibility with ISO14001:2004.&lt;br /&gt;A tool for assessing the impacts versus benefits for proposed changes was created to assist the drafters of the amendment in deciding which changes should be included, and to assist in the verification of drafts against the identified user needs. There are wo particular areas in which government action would be useful:(a) providing information on he sectors and markets where &lt;a title="ISO 14001" href="http://www.iso14000store.com/" rel="nofollow"&gt;ISO 14001&lt;/a&gt; certification s a significant issue and assisting sector rganizations to develop appropriate responses, and(b) helping to establish a certification framework, ased on strengthening national standards organizations and encouraging competitive private sector provision of auditing and certificationservices. At present, the World Bank is having discussions with a number of countries about how assistance could be provided with these issues.&lt;br /&gt;Governments should see EMS approaches as part of a broad environmental strategy that includes regulatory systems, appropriate financial incentives, and encouragement of improved industrial performance. Such encouragement can really only be effective where there is cooperation at the government level between the relevant departments, including industry and trade, as well as environment. The emphasis tended to be placed on conformance with procedures rather than the overall process of management—which was likely the actual intent.ISO 9000:19942000 version&lt;br /&gt;emphasized quality assurance via preventive actions, instead of just checking final product, and continued to require evidence of compliance with documented procedures. As with the first edition, the down-side was that companies tended to implement its requirements by creating shelf-loads of procedure manuals, and becoming burdened with an ISO bureaucracy. In some companies, adapting and improving processes could actually be impeded by the quality system.ISO 9001:2000&lt;br /&gt;The ISO 9000 standard is continually being revised by standing technical committees and advisory groups, who receive feedback from those professionals who are implementing the standard.2008 version&lt;br /&gt;ISO 9001:2008 only introduces clarifications to the existing requirements of ISO 9001:2000 and some changes intended to improve consistency with ISO 14001:2004. A Practical Guide to ISO 9001:2008 Implementation A quality management system being upgraded just needs to be checked to see if it is following the clarifications introduced in the amended version.&lt;br /&gt;combines the three standards 9001, 9002, and 9003 into one, called 9001. Design and development procedures are required only if a company does in fact engage in the creation of new products. The 2000 version sought to make a radical change in thinking by actually placing the concept of process management front and center (”Process management” was the monitoring and optimizing of a company’s tasks and activities, instead of just inspecting the final product). The 2000 version also demands involvement by upper executives, in order to integrate quality into the business system and avoid delegation of quality functions to junior administrators. Another goal is to improve effectiveness via process performance metrics — numerical measurement of the effectiveness of tasks and activities. This is what makes &lt;a title="ISO 9000 Standard" href="http://www.iso9001store.com/" rel="nofollow"&gt;ISO 9000&lt;/a&gt; different from other standards: it is a management standard, not a product standard. It goes beyond product standardisation: it is standardising not what is made but how it is made. To use standards to dictate and control how organisations work was to extend the role of standards to new territory. To take such a step we might have firstly established that any such requirements worked — that they resulted in ways of working which improved performance.&lt;br /&gt;Yet the plausibility of this Standard, and the fact that those who had an interest in maintaining it were (and still are) leading opinion, prevented such enquiries. In simple terms the Standard asks managers to say what they do, do what they say and prove it to a third party.&lt;br /&gt;&lt;a title="ISO 9001" href="http://www.iso14000store.com/" rel="nofollow"&gt;ISO 9000 (1994)&lt;/a&gt; paragraph 1: “The requirements specified are aimed primarily at achieving customer satisfaction by preventing non-conformity at all stages from design through servicing.”&lt;br /&gt;To put it another way, the Standard asserts that preventing non-conformance achieves customer satisfaction. But does it? Of course it matters to customers that a product works. But there is no guarantee that the Standard will ensure even that. Furthermore, customers take a total view of an organisation — how easy it is to do business with — in respect of all things of importance to each and every customer.&lt;br /&gt;ISO 9000 requires managers to “establish and maintain a documented quality system as a means of ensuring that product conforms to specified requirements”. Loosely translated this is “say what you do”. Management is supposed to “define and document its policy for quality . . . including its commitment to quality”.&lt;br /&gt;What management would not declare its commitment to quality? But would they know what it means? Would they argue (as they should) that quality management is a different and better way to do business, or would they believe that ISO 9000 will take care of quality? The Standard encourages managers to think of “quality” and “business as usual” as separate and distinct. It helps managers avoid the revelation that quality means a wholly different view of management. Instead, the organisation “shall appoint a management representative who, irrespective of other responsibilities, shall have defined authority and responsibility” [for ISO 9000]. At a practical level this means only one executive might decide he or she had better learn a thing or two about quality. However, would being responsible for ISO 9000 lead to learning about quality or simply enforcing the ISO 9000 regime in an organisation?&lt;br /&gt;Key to the regime is auditing. The Standard requires organisations to conduct internal quality audits to “verify whether quality activities comply with planned arrangements”. This can be loosely translated as “do you do as you say?” and the purpose of the audit is to see that you do. It was not until the 1994 review that the words were changed to “quality activities and related results”. It was a Standard which was rooted in the philosophy of inspection: fifteen years after its initial promulgation the promoters sought to extend the focus to results. But results or improvements assessed by what means? Inspection. By the time the Standard was adopted world-wide, quality thinking had moved a long way from the philosophy of inspection. It is now understood, at least by a few, that quality is achieved through managing the organisation as a system and using measures which enable managers to improve flow and reduce variation (which we explore in chapters 5 and 7). The defenders argue that there is nothing stopping a company having ISO 9000 and implementing methods for managing flow and reducing variation, but where are such companies? Few of the companies we researched, formally and informally, knew anything about this thinking. The Standard does not talk about it; moreover, the Standard effectively discourages managers from learning about it by representing quality in a different way.&lt;br /&gt;According to ISO 8402 (quality vocabulary), quality is:&lt;br /&gt;“The totality of features and characteristics of a product or service that bear on its ability to satisfy stated or implied needs.”&lt;br /&gt;Everything we have learned about &lt;a title="ISO 9000 Operating Procedure" href="http://www.iso-consults.com/" rel="nofollow"&gt;ISO 9000&lt;/a&gt; suggests that the people who created this definition were thinking about the things which need to be controlled, those things which “bear on its ability . . .”. The builders of the Standard assumed that customer needs would be listed in contractual agreements between the supplier and customer.  ISO 9000 has a “make” logic — procedures for “how you do what you do” — and a “control” logic — check to see that it is done. It is a relic of the era when contractual agreements were perceived to be an important device for regulating the behaviour of suppliers. In these ways, &lt;a title="ISO 9000 Template" href="http://www.quality-template.com/" rel="nofollow"&gt;ISO 9000&lt;/a&gt; encouraged “planning for quality”.&lt;br /&gt;Planning for quality sounds plausible, but it assumes many things: that the plan is the right plan, that it is feasible, that people will “do it”, that performance will improve. It is an approach which, paradoxically, leads to poor decisions. Planners of quality systems, guided by ISO 9000, start with a view of how the world should be as framed by the Standard. It&lt;br /&gt;was believed that if organizations were able to demonstrate they were&lt;br /&gt;operating a quality system that met international standards, customers would&lt;br /&gt;gain greater confidence in the quality of products they purchased. It was also&lt;br /&gt;believed that by operating in accordance with documented procedures, errors&lt;br /&gt;would be reduced and consistency of output ensured. If you find the best way&lt;br /&gt;of achieving a result, put in place measures to prevent variation, document it&lt;br /&gt;and train others to apply it, it follows that the results produced should be&lt;br /&gt;consistently good.&lt;br /&gt;&lt;br /&gt;The requirements of the standard were perceived to be a list of things to do&lt;br /&gt;to achieve quality. The ISO co-ordinator would often draw up a plan based on&lt;br /&gt;the following logic:&lt;br /&gt;1. We have to identify resource requirements so I will write a procedure on&lt;br /&gt;identifying resource requirements&lt;br /&gt;2. We have to produce quality plans so I will write a procedure on producing&lt;br /&gt;quality plans&lt;br /&gt;3. We have to record contract review so I will write a procedure on recording&lt;br /&gt;contract reviews&lt;br /&gt;4. We have to identify design changes so I will write a procedure on identifying&lt;br /&gt;design changes&lt;br /&gt;&lt;br /&gt;The requirements in the standard were often not expressed as results to be&lt;br /&gt;achieved. Requirements for a &lt;a title="ISO 14001 Document Control System" href="http://www.e-wia.com/" rel="nofollow"&gt;documented procedure&lt;/a&gt; to be established resulted&lt;br /&gt;in just that. Invariably the objectives of the procedure were to define something&lt;br /&gt;rather than to achieve something. This led to documentation without any clear&lt;br /&gt;purpose that related to the achievement of quality. Those producing the&lt;br /&gt;documentation were focusing on meeting the standard not on achieving quality.&lt;br /&gt;Those producing the product were focusing on meeting the customer&lt;br /&gt;requirement but the two were often out of sync. As quality assurance became&lt;br /&gt;synonymous with procedures, so people perceived that they could achieve&lt;br /&gt;quality by following procedures. The dominance of procedures to the exclusion&lt;br /&gt;of performance is a misunderstanding of the implementers. The standard&lt;br /&gt;required a documented system that ensured product met specified requirements – a&lt;br /&gt;clear purpose. Once again the implementers lost sight of the objective. Or was it&lt;br /&gt;that they knew the objective but in order to meet it, the culture would have to&lt;br /&gt;change and if they could get the badge without doing so, why should they?&lt;br /&gt;&lt;br /&gt;Issuing a procedure was considered to equate to task completed. Unfortu-&lt;br /&gt;nately, for those on the receiving end, the procedures were filed and forgotten.&lt;br /&gt;When the auditor came around, the individual was found to be totally&lt;br /&gt;unaware of the ‘procedure’ and consequently found noncompliant with it.&lt;br /&gt;However, the auditor would discover that the individual was doing the right&lt;br /&gt;things so the corrective action was inevitably to change the procedure. The&lt;br /&gt;process of issuing procedures was not questioned, the individual concerned&lt;br /&gt;was blamed for not knowing the procedure and the whole episode failed to&lt;br /&gt;make any positive contribution to the achievement of quality. But it left the&lt;br /&gt;impression on the individual that quality was all about following procedures.&lt;br /&gt;It also left the impression that quality was about consistency and providing&lt;br /&gt;you did what you said you would do regardless of it being in the interests of&lt;br /&gt;satisfying customers, it was OK. One is left wondering whether anyone&lt;br /&gt;consulted the dictionary in which quality is defined as a degree of excellence?&lt;br /&gt;&lt;br /&gt;Another problem was that those who were to implement requirements were&lt;br /&gt;often excluded from the process. Instead of enquiring as to the best way of&lt;br /&gt;meeting a requirement, those in charge of ISO 9000 implementation assumed&lt;br /&gt;that issuing procedures would in fact cause compliance with requirements. It&lt;br /&gt;requires a study of the way work gets done to appreciate how best to meet a&lt;br /&gt;requirement. Procedures were required to be documented and the range and&lt;br /&gt;detail was intended to be appropriate to the complexity of the work, the&lt;br /&gt;methods used and the skills and training needed. The standard also only&lt;br /&gt;required work instructions where their absence would adversely affect quality.&lt;br /&gt;It is as though the people concerned did not read the requirement properly or&lt;br /&gt;had no curiosity to find out for themselves what ISO had to say about&lt;br /&gt;procedures – they were all too ready to be told what to do without questioning&lt;br /&gt;why they should be doing it.&lt;br /&gt;&lt;br /&gt;More often than not, the topics covered by the standard were only a sample&lt;br /&gt;of all the things that need to be done to achieve the organization’s objectives.&lt;br /&gt;The way the standard classified the topics was also often not appropriate to the&lt;br /&gt;way work was performed. As a consequence, procedures failed to be&lt;br /&gt;implemented because they mirrored the standard and not the work. &lt;a title="ISO 9000 Operating Procedure" href="http://www.iso9001store.com/" rel="nofollow"&gt;ISO 9000&lt;/a&gt;&lt;br /&gt;may have required documented procedures but it did not insist that they be&lt;br /&gt;produced in separate documents, with titles or an identification convention&lt;br /&gt;that was traceable to the requirements.&lt;br /&gt;&lt;br /&gt;Critics argue (Seddon, John, 2000)3 that &lt;a title="ISO 9000 Software" href="http://www.iso9000-software.com/" rel="nofollow"&gt;ISO 9000 &lt;/a&gt;did not enable organiza-&lt;br /&gt;tions to reduce variation as a result of following the procedures. It is true that&lt;br /&gt;ISO 9000 did not explain the theory of variation – it could have done, but&lt;br /&gt;perhaps it was felt that this was better handled by the wealth of literature&lt;br /&gt;available at the time. However, &lt;a title="ISO 9000 Quality Management" href="http://www.iso14000store.com/" rel="nofollow"&gt;ISO 9000&lt;/a&gt; did require organizations to identify&lt;br /&gt;where the use of statistical techniques was necessary for establishing,&lt;br /&gt;controlling and verifying process capability but this was often misunderstood.&lt;br /&gt;Clause 4.14 of ISO 9001 required corrective action procedures – procedures to&lt;br /&gt;identify variation and eliminate the cause so this should have resulted in a&lt;br /&gt;reduction in variation. The procedures did not always focus on results – they&lt;br /&gt;tended to focus on transactions – sending information or product from A to B.&lt;br /&gt;The concept of corrective action was often misunderstood. It was believed to be&lt;br /&gt;about fixing the problem and preventive action was believed to be about&lt;br /&gt;preventing recurrence. Had users read ISO 8402 they should have been&lt;br /&gt;enlightened. Had they read Deming they would have been enlightened but in&lt;br /&gt;many cases the language of ISO 9000 was a deterrent to learning. Had the&lt;br /&gt;auditors understood variation, they too could have assisted in clarifying these&lt;br /&gt;issues but they too seemed ignorant – willing to regard clause 4.20 as not&lt;br /&gt;applicable in many cases.&lt;br /&gt;&lt;br /&gt;Clause 4.6 of the undervalued and forgotten standard ISO 9000 –1 starts with&lt;br /&gt;‘The International Standards in the ISO 9000 family are founded upon the&lt;br /&gt;understanding that all work is accomplished by a process.’ In clause 4.7 it starts&lt;br /&gt;with ‘Every organization exists to accomplish value-adding work. The work is&lt;br /&gt;accomplished through a network of processes’ In clause 4.8 it starts with ‘It is&lt;br /&gt;conventional to speak of quality systems as consisting of a number of elements.&lt;br /&gt;The quality system is carried out by means of processes which exist both within&lt;br /&gt;and across functions’ Alas, few people read ISO 9000–1 and as a result the&lt;br /&gt;baggage that had amassed was difficult to shed especially because there were&lt;br /&gt;few if any certification bodies suggesting that the guidance contained in ISO&lt;br /&gt;9000 –1 should be applied. Unfortunately, this message from ISO 9000 –1 was not&lt;br /&gt;conveyed through the requirements of ISO 9001. ISO 9001 was not intended as&lt;br /&gt;a design tool. When&lt;br /&gt;dealing with products the characteristics are almost always technical character-&lt;br /&gt;istics, whereas service quality characteristics have a human dimension. Some&lt;br /&gt;typical quality characteristics are given below.&lt;br /&gt;&lt;br /&gt;Product characteristics&lt;br /&gt;1. Accessibility Functionality Size&lt;br /&gt;2. Availability Interchangeability Susceptibility&lt;br /&gt;3. Appearance Maintainability Storability&lt;br /&gt;4. Adaptability Odour – Strength&lt;br /&gt;5. Cleanliness Operability -Taste&lt;br /&gt;6. Consumption Portability – Testability&lt;br /&gt;7. Durability Producibility Traceability&lt;br /&gt;8. Disposability Reliability – Toxicity&lt;br /&gt;9. Emittance Reparability Transportability&lt;br /&gt;10. Flammability Safety –  Vulnerability&lt;br /&gt;11. Flexibility Security –  Weight&lt;br /&gt;&lt;br /&gt;Service quality characteristics&lt;br /&gt;1. Accessibility Credibility – Honesty&lt;br /&gt;2. Accuracy Dependability Promptness&lt;br /&gt;3. Courtesy Efficiency –  Responsiveness&lt;br /&gt;4. Comfort Effectiveness Reliability&lt;br /&gt;5. Competence Flexibility – Security&lt;br /&gt;&lt;br /&gt;These are the characteristics that need to be specified and their achievement&lt;br /&gt;controlled, assured, improved, managed and demonstrated. These are the&lt;br /&gt;characteristics that form the subject matter of the product requirements&lt;br /&gt;referred to in &lt;a title="ISO 9001" href="http://www.iso-consults.com/" rel="nofollow"&gt;ISO 9000&lt;/a&gt;. When the value of these characteristics is quantified or&lt;br /&gt;qualified they are termed product requirements. We used to use the term quality&lt;br /&gt;requirements but this caused a division in thinking that resulted in people&lt;br /&gt;regarding quality requirements as the domain of the quality personnel and&lt;br /&gt;technical requirements being the domain of the technical personnel. All&lt;br /&gt;requirements are quality requirements – they express needs or expectations that&lt;br /&gt;are intended to be fulfilled by a process output that possesses inherent&lt;br /&gt;characteristics. We can therefore drop the word quality. If a modifying word is&lt;br /&gt;needed in front of the word requirements it should be a word that signifies the&lt;br /&gt;subject of the requirements. Transportation system requirements would be&lt;br /&gt;requirements for a transportation system, Audio speaker design requirements&lt;br /&gt;would be requirements for the design of an audio speaker, component test&lt;br /&gt;requirements would be requirements for testing components, and management&lt;br /&gt;training requirements would be requirements for training managers. ISO 9000&lt;br /&gt;requirements are often referred to as quality requirements as distinct from other&lt;br /&gt;types of requirements but this is misleading. &lt;a title="ISO 9000 Quality Management" href="http://www.e-wia.com/" rel="nofollow"&gt;ISO 9000&lt;/a&gt; is no more a quality&lt;br /&gt;requirement than is ISO 1000 on SI units, ISO 2365 for Ammonium nitrate or&lt;br /&gt;ISO 246 for Rolling Bearings. One views &lt;a title="Quality Management System" href="http://www.e-wia.com/" rel="nofollow"&gt;quality&lt;/a&gt;&lt;br /&gt;&lt;a title="Quality Management System" href="http://www.e-wia.com/" rel="nofollow"&gt;management &lt;/a&gt;as the management of success and the other the elimination of&lt;br /&gt;failure. They are both valid. Each approaches the subject from a different&lt;br /&gt;angle:&lt;br /&gt;&lt;br /&gt;The ‘success’ school is characterized by five questions (Hoyle, David and&lt;br /&gt;Thompson, John, 2001)3 :&lt;br /&gt;1 What are you trying to do?&lt;br /&gt;2 How do you make it happen?&lt;br /&gt;3 How do you know it’s right?&lt;br /&gt;4 How do you know it’s the best way of doing it?&lt;br /&gt;5 How do you know it’s the right thing to do?&lt;br /&gt;&lt;br /&gt;The ‘failure elimination’ school is characterized by five different questions&lt;br /&gt;1 How do you know what is needed?&lt;br /&gt;2 What could affect your ability to do it right?&lt;br /&gt;3 What checks are made to verify achievement?&lt;br /&gt;4 How do you ensure the integrity of these checks?&lt;br /&gt;5 What action is taken to prevent a recurrence of failure?&lt;br /&gt;&lt;br /&gt;In an ideal world, if we could design products, services and processes that&lt;br /&gt;could not fail we would have achieved the ultimate goal. Success means not&lt;br /&gt;only that products, services and processes fulfil their function but also that the&lt;br /&gt;function is what customers’ desire. Failure means not only that products,&lt;br /&gt;services and processes would fail to fulfil their function but also that their&lt;br /&gt;function was not what customers desired. A gold-plated mousetrap that does&lt;br /&gt;not fail is not a success if no one needs a gold-plated mousetrap!&lt;br /&gt;The introductory clause of &lt;a title="ISO 9001" href="http://www.iso-consults.com/" rel="nofollow"&gt;ISO 9001:1994&lt;/a&gt; contained a statement that the aim&lt;br /&gt;of the requirements is to achieve customer satisfaction by prevention of&lt;br /&gt;nonconformities. (This was indicative of the failure school of thought.) The&lt;br /&gt;introductory clause of ISO 9001:2000 contains a statement that the aim is to&lt;br /&gt;enhance customer satisfaction through the effective application of the quality&lt;br /&gt;management system and the assurance of conformity to customer and&lt;br /&gt;applicable regulatory requirements. (This is indicative of the success school of&lt;br /&gt;thought.)&lt;br /&gt;&lt;br /&gt;In reality you cannot be successful unless you know of the risks you are&lt;br /&gt;taking and plan to eliminate, reduce or control them. A unification of these&lt;br /&gt;approaches is what is therefore needed for organizations to achieve, sustain&lt;br /&gt;and improve quality. You therefore need to approach the achievement of&lt;br /&gt;quality from two different angles and answer two questions. What do we need&lt;br /&gt;to do to succeed and what do we need to do to prevent failure?&lt;br /&gt; &lt;br /&gt;Quality does not appear by chance, or if it does it may not be repeated. One&lt;br /&gt;has to design quality into the products and services. It has often been said that&lt;br /&gt;one cannot inspect quality into a product. A product remains the same after&lt;br /&gt;inspection as it did before, so no amount of inspection will change the quality&lt;br /&gt;of the product. However, what inspection does is measure quality in a way that&lt;br /&gt;allows us to make decisions on whether or not to release a piece of work. Work&lt;br /&gt;that passes inspection should be quality work but inspection unfortunately is&lt;br /&gt;not 100% reliable. Most inspection relies on human judgement and this can be&lt;br /&gt;affected by many factors, some of which are outside our control (such as the&lt;br /&gt;private life, health or mood of the inspector). We may also fail to predict the&lt;br /&gt;effect that our decisions have on others. Sometimes we go to great lengths in&lt;br /&gt;preparing organization changes and find to our surprise that we neglected&lt;br /&gt;something or underestimated the effect of something. It also emphasizes that the extent of the QMS documentation may differ from one organization to another due to:&lt;br /&gt;the size of organization and type of activities;&lt;br /&gt;the complexity of  processes and their interactions, and&lt;br /&gt;the competence of personnel.&lt;br /&gt;All the documents that form part of the QMS have to be controlled in accordance with clause 4.2.3 of ISO 9001:2008, or, for the particular case of records, according to clause 4.2.4.&lt;br /&gt;Guidance on Clause 4.2 of &lt;a title="ISO 9001:2008" href="http://www.iso9001store.com/" rel="nofollow"&gt;ISO 9001:2008 &lt;/a&gt;&lt;br /&gt;The following comments are intended to assist users of ISO 9001:2008 in understanding the intent of the general documentation requirements of the International Standard.&lt;br /&gt;a)    Documented statements of a quality policy and objectives:&lt;br /&gt;Requirements for the quality policy are defined in clause 5.3 of ISO 9001:2008. The documented quality policy has to be controlled according to the requirements of clause 4.2.3.Note: Organizations that are revising their quality policy for the first time, or in order to meet the amended requirements in ISO 9001:2008, should pay particular attention to clause 4.2.3 (c), (d) and (g).&lt;br /&gt;Requirements for quality objectives are defined in clause 5.4.1 of ISO 9001:2008. These documented quality objectives are also subject to the document control requirements of clause 4.2.3.&lt;br /&gt;b)    Quality Manual:&lt;br /&gt;Clause 4.2.2 of ISO 9001:2008 specifies the minimum content for a &lt;a title="Quality Manual" href="http://www.iso-consults.com/" rel="nofollow"&gt;quality manual&lt;/a&gt;. The format and structure of the manual is a decision for each organization, and will depend on the organization’s size, culture and complexity. Some organizations may choose to use the &lt;a title="Quality Manual" href="http://www.e-wia.com/" rel="nofollow"&gt;quality manual&lt;/a&gt; for other purposes besides that of simply documenting the QMS&lt;br /&gt;A small organization may find it appropriate to include the description of its entire QMS within a single manual, including all the &lt;a title="Documentation Procedures" href="http://www.iso-consults.com/" rel="nofollow"&gt;documented procedures&lt;/a&gt; required by the standard.&lt;br /&gt;Large, multi-national organizations may need several manuals at the global, national or regional level, and a more complex hierarchy of documentation.&lt;br /&gt;The quality manual is a document that has to be controlled in accordance with the requirements of clause 4.2.3.&lt;br /&gt;c)    Documented procedures:&lt;br /&gt;ISO 9001:2008 specifically requires the organization to have “documented procedures” for the following six activities:4.2.3 Control of documents4.2.4 Control of records8.2.2 Internal audit8.3 Control of nonconforming product8.5.2 Corrective action8.5.3 Preventive action&lt;br /&gt;These documented procedures have to be controlled in accordance with the requirements of clause 4.2.3&lt;br /&gt;Some organizations may find it convenient to combine the procedure for several activities into a single documented procedure (for example, corrective action and preventive action). Others may choose to document a given activity by using more than one documented procedure (for example, internal audits). Both are acceptable.&lt;br /&gt;Some organizations (particularly larger organizations, or those with more complex processes) may require additional documented procedures (particularly those relating to product realization processes) to implement an effective QMS.&lt;br /&gt;Other organizations may require additional procedures, but the size and/or culture of the organization could enable these to be effectively implemented without necessarily being documented. However, in order to demonstrate compliance with ISO 9001:2008, the organization has to be able to provide objective evidence (not necessarily documented) that its QMS has been effectively implemented.&lt;br /&gt;d)    Documents needed by the organization to ensure the effective planning, operation and control of its processes:&lt;br /&gt;In order for an organization to demonstrate the effective implementation of its QMS, it may be necessary to develop documents other than documented procedures. However, the only documents specifically mentioned in ISO 9001:2008 are:- Quality policy (clause 4.2.1.a)- Quality objectives (clause 4.2.1.a)- Quality manual (clause 4.2.1.b)&lt;br /&gt;There are several requirements of &lt;a title="ISO 9001:2008" href="http://www.e-wia.com/" rel="nofollow"&gt;ISO 9001:2008&lt;/a&gt; where an organization could add value to its QMS and demonstrate conformity by the preparation of other documents, even though the standard does not specifically require them. They are generated during&lt;br /&gt;which time they acquire an identity and are then&lt;br /&gt;assigned for storage for a prescribed period. During&lt;br /&gt;use and storage they need to be protected from&lt;br /&gt;inadvertent or malicious destruction and as they&lt;br /&gt;may be required to support current activities or&lt;br /&gt;investigations, they need to be brought out of storage&lt;br /&gt;quickly. When their usefulness has lapsed, a decision is made as to whether to&lt;br /&gt;retain them further or to destroy them.&lt;br /&gt;Readily retrievable means that records can be obtained on demand within a&lt;br /&gt;reasonable period (hours not days or weeks) Readily identifiable means that&lt;br /&gt;the identity can be discerned at a glance.&lt;br /&gt;Although the requirement implies a single procedure, several may be&lt;br /&gt;necessary because there are several unconnected tasks to perform. A procedure&lt;br /&gt;cannot in fact ensure a result. It may prescribe a course of action which if&lt;br /&gt;followed may lead to the correct result, but it is the process that ensures the&lt;br /&gt;result not the procedure.&lt;br /&gt;The revised requirement omits several aspects covered in clause 4.16 of the&lt;br /&gt;1994 version.&lt;br /&gt;Collection of records is now addressed by Analysis of data (clause 8.4)&lt;br /&gt;Indexing of records is a specific form of identification and is therefore&lt;br /&gt;already addressed&lt;br /&gt;Access is now addressed by the requirement for record retrieval&lt;br /&gt;Filing is a specific form of storage and is therefore already addressed&lt;br /&gt;You may only need one procedure which covers all the requirements but this&lt;br /&gt;is not always practical. The provisions you make for specific records should be&lt;br /&gt;included in the documentation for controlling the activity being recorded. For&lt;br /&gt;example, provisions for inspection records should be included in the inspection&lt;br /&gt;procedures; provisions for design review records should be included in the&lt;br /&gt;design review procedure. Within such procedures you should provide the&lt;br /&gt;forms (or content requirement for the records), the identification, collection/&lt;br /&gt;submission provisions, the indexing and filing provisions. It may be more&lt;br /&gt;practical to cover the storage, disposal and retention provisions in separate&lt;br /&gt;procedures because they may not be type-dependent. Where each department&lt;br /&gt;retains their own records, these provisions may vary and therefore warrant&lt;br /&gt;separate procedures.&lt;br /&gt;Unlike prescriptive documents, records may contain handwritten elements and&lt;br /&gt;therefore it is important that the handwriting is legible. If this becomes a&lt;br /&gt;problem, you either improve discipline or consider electronic data capture.&lt;br /&gt;Records also become soiled in a workshop environment so may need to be&lt;br /&gt;protected to remain legible. With electronically captured data, legibility is often&lt;br /&gt;not a problem. However, photographs and other scanned images may not&lt;br /&gt;transfer as well as the original and lose detail so care has to be taken in&lt;br /&gt;selecting appropriate equipment for this task.&lt;br /&gt;Whatever the records, they should carry some&lt;br /&gt;identification in order that you can determine what&lt;br /&gt;they are, what kind of information they record and&lt;br /&gt;what they relate to. A simple way of doing this is to&lt;br /&gt;give each record a reference number and a name or&lt;br /&gt;title in a prominent location on the record.&lt;br /&gt;&lt;br /&gt;1994 –2000 Differences&lt;br /&gt;&lt;br /&gt;Previously the standard&lt;br /&gt;covered retrieval in four&lt;br /&gt;ways. It required:&lt;br /&gt;(a) that quality records be&lt;br /&gt;&lt;br /&gt;made available for&lt;br /&gt;evaluation by the&lt;br /&gt;customer or his&lt;br /&gt;representative for an&lt;br /&gt;agreed period, where&lt;br /&gt;agreed contractually&lt;br /&gt;&lt;br /&gt;Records can take various forms – reports contain-&lt;br /&gt;&lt;br /&gt;ing narrative, computer data, and forms containing&lt;br /&gt;data in boxes, graphs, tables, lists and many others.&lt;br /&gt;Where forms are used to collect data, they should&lt;br /&gt;carry a form number and name as their identifica-&lt;br /&gt;tion. When completed they should carry a serial&lt;br /&gt;number to give each a separate identity. Records&lt;br /&gt;should also be traceable to the product or service&lt;br /&gt;they represent and this can be achieved either within&lt;br /&gt;the reference number or separately, provided that the&lt;br /&gt;chance of mistaken identity is eliminated. On&lt;br /&gt;defence contracts the contractor has to keep records for up to 20 years and&lt;br /&gt;for product liability purposes, in the worst-case situation (taking account of&lt;br /&gt;appeals) you could be asked to produce records up to 17 years after you&lt;br /&gt;made the produc&lt;br /&gt;The period between management system assessments – assessors may wish to see&lt;br /&gt;evidence that corrective actions from the last assessment were taken. If the&lt;br /&gt;period of assessment is three years and you dispose of the evidence after 2&lt;br /&gt;years, you will have some difficulty in convincing the assessor that you&lt;br /&gt;corrected the deficiency.&lt;br /&gt;While the ISO 9001 requirement applies only to records, you may also need to&lt;br /&gt;retain tools, jigs, fixtures, test software – in fact anything that is needed to repair&lt;br /&gt;or reproduce equipment in order to honour your long-term commitments.&lt;br /&gt;Should the customer specify a retention period greater than what you&lt;br /&gt;prescribe in your procedures, special provisions will need to be made and this&lt;br /&gt;is a potential area of risk. Customers may choose not to specify a particular&lt;br /&gt;time and require you to seek approval before destruction. Any contract that&lt;br /&gt;requires you to do something different creates a problem in conveying the&lt;br /&gt;requirements to those who are to implement them. The simple solution is to&lt;br /&gt;persuade your customer to accept your policy. You may not want to change&lt;br /&gt;your procedures for one contract. If you can’t change the contract, the only&lt;br /&gt;alternative is to issue special instructions. The requirement should not be confused with that on the retention&lt;br /&gt;of records. Retention times are one thing and disposal procedures quite&lt;br /&gt;another.&lt;br /&gt;The standard does not specifically require records to be authenticated, certified&lt;br /&gt;or validated other than product verification records in clause 8.2.4. A set of&lt;br /&gt;results without being endorsed with the signature of the person who captured&lt;br /&gt;them or other authentication lacks credibility. Facts that have been obtained by&lt;br /&gt;whatever means should be certified for three reasons:&lt;br /&gt;&lt;br /&gt;They provide a means of tracing the result to the originator in the event of&lt;br /&gt;problems.&lt;br /&gt;They indicate that the provider believes them to be correct.&lt;br /&gt;They enable you to verify whether the originator was appropriately&lt;br /&gt;qualified.&lt;br /&gt;They give the results credibility.&lt;br /&gt;&lt;br /&gt;If the records are generated by computer and retained in computerized form,&lt;br /&gt;a means needs to be provided for the results to be authenticated. If the facts change, a new record is usually&lt;br /&gt;created rather than the previous record revised. Even&lt;br /&gt;where a record is revised and new facts added, the&lt;br /&gt;old facts remain identified as to their date. The only reason for revising facts&lt;br /&gt;contained in a record without changing the identity of the record or the date&lt;br /&gt;when they were collected is where the facts were incorrectly recorded. This&lt;br /&gt;subtle difference demands different treatment for documents that are classed&lt;br /&gt;as records to those that are classed as informative. As with other types of&lt;br /&gt;documents, records result from processes and may be used as inputs to other&lt;br /&gt;processes.&lt;br /&gt;Records required by the management system means records used or&lt;br /&gt;generated by the management system. There is therefore no requirement to&lt;br /&gt;produce records solely to satisfy an auditor. The records required are those for&lt;br /&gt;the effective operation of the organization’s processes (see clause 4.1d of ISO&lt;br /&gt;9001). If a record has no useful purpose within the management system there&lt;br /&gt;is no requirement that it be established or maintained.&lt;br /&gt;This does not mean that a record is required to prove conformity with every&lt;br /&gt;single requirement for every product and service. There are those records&lt;br /&gt;required by the standard (see below) that are required for every product and&lt;br /&gt;service where applicable and in all other cases, audit records showing&lt;br /&gt;compliance on a sample of operations would be sufficient.&lt;br /&gt;Compliance with many requirements can be demonstrated by observation,&lt;br /&gt;analysis, interview or examination of documentation. Evaluate the organization’s need/goals for implementing a QMS Need may arise from repeated customer complaints; frequent warranty returns; delayed deliveries; high inventories; frequent production hold-ups; and high level of rework or rejection of products or services.&lt;br /&gt;At this stage, identify the goals which you would like to achieve through a &lt;a title="QMS" href="http://www.iso-consults.com/" rel="nofollow"&gt;QMS&lt;/a&gt;, such as customer satisfaction, increased market share, improved communications and morale in the organization, greater efficiency and profitability, etc. Another objective in implementing a QMS may be a demonstration of compliance through third party certification, which may be requested by an important client or required for enlisting as a supplier to large companies, e.g., original equipment manufacturers (OEMs).&lt;br /&gt;2. Obtain information about the &lt;a title="ISO 9000 family" href="http://www.iso14000store.com/" rel="nofollow"&gt;ISO 9000 family&lt;/a&gt;&lt;br /&gt;The persons identified for initiating the development of an &lt;a title="ISO 9000 QMS" href="http://www.iso9000-software.com/" rel="nofollow"&gt;ISO 9000 QMS&lt;/a&gt; need tounderstand the requirements of ISO 9001:2008 as read with ISO 9000:2000 and ISO 9001:2008.&lt;br /&gt;Supporting information such as quality management principles, frequently asked questions (FAQs), guidance on clause 1.2 (application) of ISO 9001:2008, guidance on documentation requirements of ISO 9001:2008 and other brochures are available free of charge on the ISO web site;&lt;br /&gt;3. Appoint a consultant, if necessary&lt;br /&gt;If, within the organization, you do not have adequate competence to develop a QMS, you may appoint a consultant. Before doing so, it is good to check his/her background; knowledge about the product realization processes of your organization; and experience in helping other organizations to achieve their stated goals, including certification.&lt;br /&gt;Carry out a cost-benefit analysis of hiring a consultant and agree the scope of his/her work in writing. It is also possible to appoint a consultant only for the training of key staff; the latter can then carry out further training and development of the system.&lt;br /&gt;4. Awareness and training&lt;br /&gt;Raise awareness about QMS requirements amongst all personnel performing activities that affect quality. Plan for and provide specific training on how to develop Quality Manuals; on procedures; on QMS planning; on how to identify and implement improvement processes; and on how to audit compliance with the QMS, etc.&lt;br /&gt;The Institute of Quality Assurance (IQA), the American Society for Quality (ASQ)and the International Auditor and Training Certification Association (IATCA) can provide lists of training organizations.&lt;br /&gt;5. Gap analysis&lt;br /&gt;Evaluate gaps between your existing &lt;a title="Quality Management System" href="http://www.iso9001-standard.us/"&gt;quality management system&lt;/a&gt; and the QMS requirements of ISO 9001. Prepare how to bridge these gaps, including by planning for any additional resources required. Gap analysis may be carried out through selfassessment or by the external consultant.&lt;br /&gt;6. Product realization processes&lt;br /&gt;Review clause 7 of ISO 9001:2008 relating to “Product realization” to determine how the requirements apply or do not apply to your company’s QMS.&lt;br /&gt;The processes covered by this clause include:&lt;br /&gt;• Customer-related processes&lt;br /&gt;• Design and development&lt;br /&gt;• Purchasing&lt;br /&gt;• Production and service provision&lt;br /&gt;• Control of measuring and monitoring devices&lt;br /&gt;Note that if your company is not responsible for preparing the design of your product, you can exclude the requirement for “design and development” from your QMS and explain the reasons for doing so in your Quality Manual.&lt;br /&gt;7. Staffing&lt;br /&gt;Decide on the responsibilities of the persons who will be involved in developing and documenting the QMS, including the appointment of a management representative who will oversee the implementation of the &lt;a title="QMS" href="http://www.e-wia.com/" rel="nofollow"&gt;QMS&lt;/a&gt;. Establishing a project Steering Committee may also prove useful to oversee progress and provide resources wherever required.&lt;br /&gt;8. Planning a time frame&lt;br /&gt;Prepare a complete plan to close the gaps identified in Step 5 to develop the &lt;a title="QMS" href="http://www.iso%209001store.com/" rel="nofollow"&gt;QMS&lt;/a&gt; processes. In the plan, include activities to be performed, resources required, responsibilities and an estimated completion time for each activity. Clauses 4.1 and&lt;br /&gt;7.1 of &lt;a title="iso 9001:2008" href="http://www.iso9001-standard.us/"&gt;ISO 9001:2008 &lt;/a&gt;provide information that should be used when developing the plan. The total time required for each phase (planning, documentation, implementation and evaluation) depends on the extent of the gaps in your existing QMS.&lt;br /&gt;9. Draft a Quality Manual&lt;br /&gt;In your Quality Manual;&lt;br /&gt;• Include how the QMS applies to the products, processes, locations and departments of the organization;&lt;br /&gt;• Exclude any requirement with justification for doing so as decided in step 6&lt;br /&gt;above;&lt;br /&gt;• Refer to or include documented procedures for &lt;a title="QMS" href="http://www.quality-template.com/" rel="nofollow"&gt;QMS&lt;/a&gt;;&lt;br /&gt;• Describe the interaction between the processes of the QMS, e.g., the interaction between product realization processes and other management, measurement and improvement processes; and&lt;br /&gt;• Draft the quality policy and quality objectives for the organization.&lt;br /&gt;The staff concerned in the organization should review the Quality Manual and the documented procedures so that their comments and suggestions can be taken into account before the Quality Manual and procedures are approved for issue and use.&lt;br /&gt;The effective date of implementation should also be decided.&lt;br /&gt;10. Carry out internal audits&lt;br /&gt;During the phase of implementation of some three to six months after the documentation has been written, the trained auditors should carry out one or two internal audits covering all activities for the QMS, and concerned management should take corrective action on the audit findings without delay. Wherever required, revise the manuals, procedures and objectives. After each internal audit, the top management should review the effectiveness of the system and provide necessary resources for corrective actions and improvements.&lt;br /&gt;11. Apply for certification&lt;br /&gt;On satisfactory completion of Step 10, and if your company decides to obtain third party certification, you can make an application for certification to an accredited certification body. The certification audit process is explained section VII.&lt;br /&gt;12. Conduct periodic evaluations&lt;br /&gt;After certification, the organization should periodically conduct internal audits to review the effectiveness of the QMS and see how it can be “continually improved”.
